Your insurance carrier requires that we see you before we
refer you to a specialist. A co-pay is charged for this visit.
Your insurance carrier also requires we see you periodically
if you are seeing a specialist on an ongoing basis. The specialist
must provide us with a treatment plan prior to authorization
of visits. It is your responsibility to keep your referral
current.
We need three business days to generate a referral. We send
the information to your insurance carrier and must wait for
a reply.
- We determine your need to see a specialist and you choose
the provider.
- You make the appointment and call our office with the
provider's name, fax number, appointment date and time,
and the reason for the appointment. Or you may complete
an online referral request form by using the link at the
bottom of this page.
- We will fax the referral to the specialist when it is
approved by the insurance carrier.
Same-day referrals can only be done on an emergency basis.
Emergencies are determined by the medical provider. Please
contact our Office Manager if you feel your situation constitutes
an emergency and you can not obtain your referral.
Additionally, insurance carriers do not allow us to back-date
referrals. It is important that you monitor your ongoing treatment
and do not allow your referral to expire.
